“The VVV (visit-to-visit variability) in real-world practice poses challenges for management of patients with hypertension based on blood pressure readings in outpatient settings and suggest the need to go beyond episodic clinic evaluation.”*
”...the more blood pressure measures you have, the more it is possible to determine whether there is meaningful real change”**
Managing chronic conditions is more effective through regular measurements of vitals. ServUs Health provides full-service chronic condition monitoring for patients and the healthcare organizations where care is delivered. ServUs RPM provides dedicated staff and full-touch, first-line monitoring and assessment for enrolled patients. We provide a multi-mode support model involving telephones, text messages, and video engagement to engage patients.
Our configurable RPM dashboard enables both population-level and patient-level ranges of “normal” for hypertension, weight monitoring, diabetes management, and oxygenation. When a patient goes outside the normal range, ServUs RPM staff provide first-line contact to assess if the primary care team needs to be alerted so intervention can be made for medication adjustments or other solutions to bring the patient back within a normal range(s)/levels.
